The EquityHealth 150 program is our service offering to small companies, service industry, and other industries that
typically do not offer a health care benefit. The employee is credited 1,500 total shares per year. They are delivered
to the employee by EquityHealth in monthly or quarterly amounts. This program will enable the employee to choose a
primary care provider for a flat annual rate, purchase an insurance plan and accumulate additional shares for future
expenses.

Employees can contribute to the plan via payroll deduction. The employee can choose to add additional shares as
well in any quantity they desire.

This is an annual agreement.
