The EquityHealth Couples 600 program is for couples that wish to share the account. Once enrolled you will be
credited 6,000 total shares per year. They are delivered by EquityHealth in monthly or quarterly amounts. This
program will enable any individual to choose a primary care provider for a flat annual rate, purchase an insurance
plan and accumulate additional shares for future expenses.
You can contribute to the plan via payroll deduction. You can choose to add additional shares as well in any quantity
desired.
This is an annual agreement.
