What is EquityHealth?
EquityHealth is a Direct to Consumer health care service broker. The company procures and brokers health care
services to our program participants, who pay the provider directly at the time of service using an internal
currency called Health Care Shares.

What are Health Care Shares?
Health Care Shares are the currency the EquityHealth Agents use to secure services for program participants.
They are redeemable for any health related expense including insurance premium payments, payments directly
to service providers, and can also be accumulated for future use, building a health security net for the account
holder. Program participants can buy shares in discounted packages or in any quantity and use them as needed.
Employers can purchase a bulk amount of shares called a Share Bank and then distribute them to their
employees on a monthly basis.

What is Open Market Health Care?
Open Market health Care is a way of offering the health care consumer an opportunity to buy services directly
from the providers that deliver them. This makes it much easier for someone to get the care they need. There are
no co-pays, deductibles, coinsurace or cost sharing, the EquityHealth program simply pays the bill, regardless of
who the provider is or what services they performed.

If I leave my job what happens to my shares?
Any shares that have been credited to your account from a plan you purchased will remain with you regardless of
your employment situation. You can continue to add shares to your account, if you do not add shares within 90
days, a prorated membership fee of 300 shares is deducted from your account. Call our help center if you need
assistance, at (1-800-717-9410)

What happens if I decide not to purchase Shares for a period of time, a month or longer?
If you do not add shares within 90 days, a prorated annual membership fee of 300 shares (25 shares a month) is
deducted from your account. Call our help center if you need assistance, at (1-800-717-9410). Your remaining
shares will be in your account and will remain active and available to you.

Any Limits on the number of shares?
There are no limits as to the number of shares that can be accumulated, one factor to consider is estimating your
short and long term usage, and plan accordingly. Our Health Care Agents can help you if needed.

Transferability.
Shares are completely transferable to anyone you choose by calling 1-800-717-9410 or through our online Care
Share program

How do I redeem my Shares?
Shares can be redeemed for any medical services, including prescription drugs and medical supplies. There is no
defined network; you can go to any provider that accepts electronic payment (similar to a credit card purchase) of
your choosing. We use PayPal for instant payments to your provider, if they have a PayPal account they can bill
us through that service.

When can I start redeeming my Shares?
Shares are redeemable once they are posted to your account.   Please remember we will be in price negotiations
with the provider you have selected. It is in your best interest to make sure you have the available number of
shares necessary to complete the transaction it also gives you greater buying power.

Expiration of Shares?
Shares do not expire they remain in your account until you redeem them.

How often can I redeem shares?                                                  
As long as you have shares in your account, you can redeem them as often as you like.

Are there a required minimum number of shares needed for redemption?
There are no minimum or maximum number of shares needed to remain active, however if you require medical
attention and you want Equity Health to take care of negotiations you will need to view the redemption options on
the website or contact a Health Care Agent at 1-800-717-9410. They can advise you if you need to add shares to
your account.

What happens if my share account is empty?
As long as your account remains active you can purchase additional shares.

Are there any restrictions on which doctor I can visit?
No there are no restrictions; we encourage you to select the doctor of your choice.

How does my share account work?
You can view your share account as being similar to a savings account for health care purposes. It continues to
grow each month until you decide to utilize it and does not have a list of complicated rules like HSA and FSA
programs.

How is my plan building health care wealth?
The plan that you bought is adding health care Shares to your account each month you pay into it. As a result
the shares are accumulating. Each year the shares that remain in your account carry over and continue to grow
with your continued payments. Over time you will build health care wealth through the accumulation of your health
care shares.

Example  
An average married couple with 2 kids will be paying roughly $7200 to $10,000 per year on a health insurance
plan. In 25yrs they will have paid out somewhere between $180,000 and $250,000 not including co-pays, and
deductibles. If they had invested all of those dollars in the Equity Health plan they could have an estimated
balance of $90,000 to $125,000 after paying for doctors and supplies. Each case will be different depending on
the health of each family member and at what age they start to accumulate shares. The point is this, the Equity
Health plan gives you the opportunity to get out of the insurance merry go round and start building your own
health care wealth instead of building someone else’s.

What happens if a provider says “we don’t take that plan”?
Since we do not formally contract with any providers it is possible a provider may be unfamiliar with your MHA
health care Share Plan. In these cases please have them call a health care Agent and we will tell them how it
works.

You can also tell them:

-        The plan is a cash based plan and they will be paid at the time of service
-        That there is no pre-agreed pricing structure and we pay similar or better rates as insurance plans such
as      Blue Cross Blue Shield, United health care and Aetna
-        There is no claim to “file” they will be paid over the phone when they call in a visit report.
If they simply refuse all of this you should find another provider by calling a health care Agent at 1-800-717-9410.

What to do if you want to use a mail order service for medications of supplies?
Call the health care Agents and they will set this up for you. You can reach them at  1-800-717-9410 or at
customerservice@equityhealthplan.com

What to do if you lose your card
Contact us immediately at 1-800-717-9410; do not email us as this will create an unacceptable delay as your
shares spend like cash. If someone fraudulently uses your membership card you will lose the shares. We will
replace your card within 14 (fourteen) working days.

What to do if you change employers
If you change employers your health care shares go with you. If your employer participated in the Care Share
program then you will lose access to the additional shares that were available in the employers share bank at the
time you leave the employer. You will need to continue to pay your monthly plan amount, if you cannot continue
to pay into your plan please call a Agent at 1-800-717-9410 and we will work something out with you until you are
employed again. You will continue to have access to and be able to use all the shares that are in your account
when you leave your employer until they have all been used.

What if you want to transfer shares to another member
You can transfer shares to anyone who also participates in the program. It does not matter how long they are
going to participate or have participated. It is ok to enroll in the program even if you only want to enroll for the
sake of transferring shares for a one time use. Our health care Agents will help you with the details, it is quick and

What to do if you want someone other than you to use the card?                
If transferring the shares is too cumbersome you can simply authorize someone to use your card directly. Simply
call the health care Agents and they will add the person as an authorized user. 1-800-717-9410.
