The EquityHealth Provider Payment Programs are a way for providers to get paid cash at the time of service, or
up front for a set of services to be delivered over time. EquityHealth accepts all providers of all types into the
EquityHealth program, the company does not require credetialing.
The Concept is based on the Medical Home Model that has been shown to improve patient care and reduce the
cost of care. There are many versions of this model but all focus on one goal, to coordinate the care of the
patient. EquityHealth builds that feature in for you. Our customers have a care coordinator all of the time we call
a Health Care Agent. All you will need to do is submit a visit report and we will take it from there.
EquityHealth Health Care Agents do not give medical advice or instruction, the simply connect patients with their
providers and make sure the data on the patient follows, Providers direct the care.

It costs nothing to participate and can be an excellent cash income generator for your practice.
The Equity Health Program Participants pay you one of two ways:
1. A single flat rate for a set of services to be delivered across one year from the date of purchase
2. A single payment for a set of delivered services
The Flat Rate program allows you to collect all the money up front.
Payouts for flat rate programs are based on the set of services available, rates here are suggestions, you can
apply any price you wish, remember you will be compared side by side with competitors, make sure you tell us all
of the good things you can offer under this format. You will receive payment in full prior to the first visit.
These prices are suggestions, providers determine their own price.

Physician Office $400 per year $300
Urgent Care (open 7 days) $550 per year $450
Urgent Care (open 5 days) $450 per year $350
In-Store Clinic (NP/PA based care) $225 per year $175
